We accept the vast majority of healthcare plans. We are a proud participant in Medicare and other Governmental plans. We also treat Workman’s Compensation. Please contact our office if you have questions about a specific plan.

new patient appointment

Please print out and complete your registration paperwork and bring it to your appointment. If you are being referred by another physician, have them fax all of your medical records to our office at (423) 664-4640.

Bring any relevant diagnostic films such as X-Ray and MRI to your appointment. Bring a picture ID, your insurance card and come prepared to pay your copay or deductible.

cancellation policy

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient.
